You should also try to find some contacts/eyes (they are used as makeup) that are realistic. You can find a lot of different contacts, but some of them aren't always looking realistic.

Sliders make a big difference, to adjust face features and structure. There are sooooo many sliders available now, and they make such a big difference. I also think the skin you use can make a big difference; some of them are very high quality and have a lot of realistic details included. As Gigi mentioned, eye contacts are a good thing to check out too and can definitely make a big difference too

I use Nraas' MasterController + Integration mods to (a) increase the amount of sliders and their intensity (x5), and (b) enable multiple layers for makeup and accessories. Then I go in CAS and that's exactly what I do, layer on all the stuff; download all the detail make up you can find. Shaders, highlights, wrinkles, freckles, etc. Sliders have already been mentioned but OneEuroMutt has a nice masterlist here. Play with the sliders, each one of them. Also, use references! Make the eyes and head smaller (when needed). EA things tend to be a bit exaggerated by default to give them a more cartoon-y appearance. Mmm, I guess that's about it really!

Yeah for me I use sliders to adjust all the features to make the sim more unique, and less 'sim like'. Then I try and get things like hair, contacts, eyebrows, lashes, lips, freckles and skin mods to just make them more realistic! But really to be honest when I edit my sims' pictures, it is just with photoshop!
